Nach dem Wahl-Blockpraktikum können die Studierenden…

Akuter Beratungsanlass: Akutkonsultationen nachvollziehen (Struktur, Priorisierung, Red Flags, Safety Netting) und das Vorgehen anhand des SOAP-Schemas beschreiben.

Chronische Erkrankung: Verlaufskontrollen (z.B. DMP) bei chronischen Erkrankungen einordnen und die Entscheidungsschritte sowie offene Fragen reflektieren.

Prävention & Früherkennung: präventive Beratungen und Früherkennung in der Sprechstunde beobachten, die zugrunde liegenden Empfehlungen benennen und die Gesprächsführung (z.B. Shared Decision Making) reflektieren.

Letzte Lebensphase (fakultativ): Betreuung von älteren Menschen oder Menschen in der letzten Lebensphase beobachtend erfassen und ethische sowie kommunikative Aspekte reflektieren.

Praxismanagement & Abläufe: zentrale Praxisabläufe beschreiben.

Interdisziplinäre Zusammenarbeit: Schnittstellenprozesse beobachten (z.B. Überweisungen, Rückfragen, Entlassmanagement, Zusammenarbeit mit Pflege-/Heilmittelerbringern), Informationswege benennen und die Rolle der Hausärzt*innen in der Versorgung reflektieren.
